Nutrition care on a chronic condition management plan

A chronic condition management plan is your GP’s way of connecting you with allied health support. If it includes a dietitian referral, Medicare covers part of up to 5 sessions a year. We confirm your out-of-pocket cost before you book, build a plan you can actually follow, and keep your GP in the loop.

Client-focused care from a team of three Accredited Practising Dietitians, so you are supported while working towards the outcomes you want.

Medicare dietitian questions

Can I see a dietitian through Medicare?

Yes. If you have a chronic health condition, your GP can prepare a chronic disease management plan (EPC plan) with a dietitian referral. Medicare rebates up to 5 sessions per calendar year.

How many Medicare dietitian sessions can I have?

Up to 5 allied health sessions per calendar year, shared across all allied health services on the plan. If you also see a physiotherapist or psychologist under the same plan, those count toward the same total.

What does a Medicare dietitian appointment cost?

The same fees as private: standard initial $145 with Shayan or Georgia and $165 with Andrew at our Ipswich clinic ($145 with Andrew elsewhere), complex initial $190, reviews $95 with Shayan or Georgia and $105 with Andrew at Ipswich ($95 elsewhere). Medicare rebates $63.40 per session, and we confirm your exact out-of-pocket cost before your first appointment. Private health extras may let you claim further on top.

What conditions qualify for Medicare dietitian sessions?

Any chronic health condition lasting or expected to last 6 months or more, such as type 2 diabetes, heart disease, chronic kidney disease, IBS, IBD, COPD, obesity, malnutrition and cancer. Your GP decides whether your condition qualifies.

Do I need a referral to see a dietitian on Medicare?

Yes, a GP referral as part of a chronic disease management plan (EPC plan). Without one, Medicare rebates do not apply, but you can still see us as a private patient.

What is a chronic disease management plan?

Now called a GP Chronic Condition Management Plan (or an earlier GPMP/TCA, also called an EPC plan), it is a plan your GP prepares when you have a chronic condition. It sets out your health goals and includes referrals to allied health professionals like dietitians. Your GP can set it up during a standard appointment.

Do you provide home visits for Medicare patients?

Yes, across Logan, Ipswich, Redlands and surrounding areas, and Medicare rebates apply to home visits just as they do in clinic or by telehealth. Telehealth is available anywhere in Australia.

Can I see a dietitian without Medicare or a referral?

Yes. You can book as a private patient at any time, with initial consultations from $145. See our private dietitian page for full pricing.
