Helping older adults eat well and live well
Unintentional weight loss is the most common reason families contact us. We screen for malnutrition, find the causes and put a plan in place, and where chewing or swallowing is hard we set up texture-modified meals aligned with IDDSI.
Your provider does not need a dietitian on their team. We work as a third-party provider under your package, your provider arranges the agreement, and we come to the house across Logan, Ipswich and Redlands.

- Type 2 diabetes in older adults, including medications
- Texture-modified diets and IDDSI for swallowing difficulties
- Tube feeding and enteral nutrition
- Heart disease, kidney disease and COPD
- Wound healing and pressure injury nutrition
- Falls prevention: protein, vitamin D, calcium and muscle strength
- Nutrition after a hospital stay, to help prevent readmission
- Meal planning and food fortification for poor appetite
- Coordinated care with GPs, speech pathologists, OTs and care teams
Malnutrition in older adults
Malnutrition is common in older Australians and often goes unrecognised. Unintentional weight loss, reduced appetite, fatigue and frequent infections can all be signs, and early support makes a real difference.

Support at Home dietitian questions
Can I get a dietitian through my Support at Home package?
Yes. Your funding can be used for dietitian services, with us working as a third-party provider through your approved provider. Book a free discovery call and we will contact your provider to arrange it.
What is a third-party provider agreement?
An arrangement where your Support at Home provider contracts an external health professional like a dietitian to deliver services under your support plan, with the cost covered by your funding. We handle the clinical side and your provider manages the agreement.
What are the signs of malnutrition in older adults?
Unintentional weight loss, clothes fitting more loosely, reduced appetite, fatigue, frequent infections, slow wound healing and difficulty concentrating. If you notice these in a family member, it is worth getting a nutrition assessment.
What is a texture-modified diet?
Food and fluids adjusted in texture so they are safer and easier to swallow for people with dysphagia. We follow the International Dysphagia Diet Standardisation Initiative (IDDSI) framework and work with speech pathologists so recommendations are safe and appropriate.
Do you provide home visits for older adults?
Yes, across Logan, Ipswich, Redlands and surrounding areas. For those who can travel, we also offer clinic appointments at Logan, Ipswich or Redlands, plus telehealth by video call.
Can you help with nutrition after a hospital stay?
Yes. Poor nutrition is a common cause of hospital readmission in older adults. We help with recovery nutrition, rebuilding strength, new dietary requirements and eating well enough to stay out of hospital.
Do you work with GPs and other health professionals?
Yes. We write to the person’s GP, speech pathologist, occupational therapist, physiotherapist and home care team after every appointment, so everyone is working toward the same goals.
Can family members be involved in appointments?
Yes, and we encourage it. Family members and carers often prepare meals and manage daily nutrition, so they are welcome at appointments and on discovery calls.
